This invitation to inquire allows eligible applicants an opportunity to inquire further about the insurance offered and is limited to a brief description of any loss for which benefits may be payable. Benefits are offered as described in the insurance contract. Benefits are subject to all deductibles, coinsurance, provisions, terms, conditions, limitations, and exclusions in the insurance contract. The contract does contain a pre-existing condition exclusion and does not cover losses or expenses related to a pre-existing condition.

Eligibility to purchase, extend or renew this product, or its terms and conditions, may be modified or amended based upon changes to applicable law, including PPACA. Please note that it is an insured person’s sole and exclusive responsibility to determine if PPACA is applicable to them. The Company shall have no liability to any person whatsoever for their failure to obtain or maintain PPACA compliant insurance coverage. Be sure to read your policy to make sure you are understand exclusions or limitations of coverage for preexisting conditions or health benefits (including hospitalization, emergency services, maternity care, preventive care, prescription drugs, and mental health and substance abuse disorder services). If this coverage expires or you lose eligibility for this coverage, you might have to wait until an open enrollment period to get other health insurance coverage.
This coverage is not "minimum essential coverage". If you don't have minimum essential coverage for any month in 2018, you may have to make a payment when you file your tax return unless you qualify for an exemption from the requirement that you have health coverage for that month. There is no such requirement in 2019.
